The Joint Replacement Home Recovery Program is designed for patients who choose to receive one-on-one rehabilitation at home under the direct supervision of the Orthopedic Surgeon. It is an individualized intensive program tailored each individual patients' needs.
  • "Standard Orders" include Physical Therapy and Nursing Care.
  • Registered Nurse and Physical Therapist will plan to visit within 24 hours of hospital discharge
  • One-on-one therapy with a personalized program tailored to the patient's and surgeon's protocol.
Program Services
Physical Therapy-
  • Assess safety and durable medical equipment needs in the home
  • Provide a written home exercise program in addition to gait training and stair management
  • Provide hands-on instruction regarding activities related to personal care and light homemaking.
Nursing-
  • Medication review
  • Wound care
  • Nutrition assessment
  • Assess of any co-morbidities.
Home Health Aide-
  • Assistance with ADL's
Medical Social Worker-
  • ​Assistance in identifying community resources

  • Optimize the patients' capabilities toward independent transfers and ambulation
  • Maximize patient and family confidence

  • Increased medication compliance
