What is it all about?
This five-week program involves skilled nurses, home health aides, physical therapists, medical social workers, and registered dietitians. Together they ensure clients are fully educated about their disease and become independent.
Who is eligible?
Any CHF client is eligible for the program. Clients can be referred through a physician's office, emergency department, rehabilitation unit, extended care, or after being discharged from an acute care facility.
What does the program include?
The CHF program promotes independence and self-monitoring through instruction in:
  • Medication management
  • Dietary compliance
  • Early recognition and intervention with onset of acute symptoms
  • Disease process and treatment building
  • Energy conservation techniques
  • Resources to support psychosocial and financial needs
  • Home adaptation
